Job Description

The post holder will be expected to work as a senior clinician collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services within an Urgent Care setting in a widely diverse locality. The Urgent Care Centre provides a 7-day a week, 24-hours a day service for 365 days a year working shifts, where appropriate and operationally required with the aim to avoid hospital admission through clinical assessment or streaming to primary care/acute services when possible. Patient needs adhere to all ages.

The post holder will be a NMC Registered Nurse responsible for the appropriate clinical streaming of patients attending the Urgent Care Centre and being professionally and legally accountable for effective streaming of patients into acute and community services.

The post holder will be able to carry out a full physical assessment including all first line diagnostics. He/she is expected to demonstrate and apply a broad range of nursing knowledge, interventions and skills. Knowledge of the principles of health promotion and preventing illness will be fundamental as will having a comprehensive understanding of the roles of others in health and social care. The post holder will lead the team for allocated shifts and provide support to the Senior Practitioner, both clinically and in terms of daily operational management.



Key Responsibilities:



1. Provide effective clinical streaming for patients entering the Urgent Care Centre to see a GP, Advanced Nurse Practitioner, Accident and Emergency services and primary care services.

2. To work as an autonomous practitioner and be professionally and legally accountable for all aspects of their professional and clinical work, within agreed national local and professional standards and guidelines and provide timely care for changing or new presentations of needs.

3. To safely and effectively refer to specialist areas within the hospital setting and being aware of pathways and policies.

4. Provide nursing care within own level of competencies for effective wound care, medication administration and observation monitoring.

5. To use enhanced skills and knowledge to assess the physical and psychosocial needs and develop a comprehensive and concordant plan of care with the patient when required.

6. Observe record and report all changes to senior clinician in a timely manner, depending on acuity and urgency.

7. Awareness of current evidence based approaches to patient care and demonstrates this in practice.

8. Give accurate and comprehensive hand over to team members at each shift change, ensuring that pertinent information, consideration, acute presentations etc, are fully disseminated to colleagues.9. Work in partnership with the patient to achieve optimum levels of self-care until primary care can review patient.
