Cheshire East Council is seeking experienced Qualified Social Workers (QSWs) to join our Adult Social Care teams.
This is an excellent opportunity for dedicated professionals to make a real impact by supporting adults to live independently, promoting well-being, and ensuring that care and support needs are met in line with the Care Act 2014.
You will work collaboratively with adults, their families, and multi-disciplinary partners to carry out strengths-based assessments, develop care plans, and review support to achieve positive outcomes.
Key Responsibilities
Undertake strengths-based assessments and develop person-centred care and support plans.
Work in partnership with health professionals, care providers, and voluntary organisations to deliver effective, coordinated services.
Manage a varied caseload of adults with complex needs, including older people, individuals with physical disabilities, and those with mental health issues.
Apply the principles of the Care Act 2014, Mental Capacity Act 2005, and Safeguarding Adults legislation.
Support hospital discharges and community-based interventions where required.
Maintain accurate and timely case records and ensure compliance with statutory requirements.
Requirements
Qualified Social Worker (Degree in Social Work, CQSW, DipSW or equivalent).
Current registration with Social Work England.
Experience within adult social care, community, or hospital-based social work.
Strong assessment, decision-making, and communication skills.
Ability to work effectively as part of a multi-disciplinary team.
Enhanced DBS check required.
