community care across Dartford, Gravesham and Swanley.





As part of our dedicated multi-disciplinary team, you'll provide exceptional palliative and end-of-life support to patients and families.

Key responsibilities:

Lead care for a defined group of patients Deliver thorough assessments and ongoing review Support families with compassion Work closely within a multi-disciplinary team Contribute to staff and student learning

Essential requirements:

Registered Nurse with 2+ years' palliative care experience Relevant degree and continual professional development Strong communication skills and MDT experience Expertise in symptom management and holistic assessment
