Community Rehabilitation Physical Health and Wellbeing Senior Recovery Navigator:
Hours: 37 hours per week
Contract: Permanent
Salary 29,691 per annum
Location Pennywell Road, Community based. Bristol
The Community Rehabilitation Service is an award winning service provided by Second Step in partnership with Avon and Wiltshire Mental Health Partnership NHS Trust and Missing Link. Shaped by people who have used mental health services, the Community Rehabilitation Service gives people the support they need to achieve their hopes and goals. We are passionate about recovery and the service is Trauma informed. The post will be based at Second Step office in Pennywell Road, Bristol.
You will work as part of a wider multi-disciplinary team delivering specialist physical health and wellbeing interventions, ensuring that services are tailored to individual's cultural and social needs.
The post holder will work with all service users on the caseload, screening their physical health needs and co-producing a care plan outlining goals and interventions agreed, the post holder will need excellent organisational and documentation skills for the role.
