This is an ideal opportunity for a current band 7 nurse to continue their development, or for a band 6 nurse with inpatient experience wanting to expand their knowledge in the area, to shape the inpatient diabetes experience for patients, families and staff at the Countess,.
It is an opportunity for someone with an enthusiasm for inpatient diabetes and a wish to extend their expertise in this field. The post will include responsibility for leading the service and planning the future care for inpatients with diabetes, using GRIFT and JBDS guidelines to push the service forward, y ou will be involved in the development and shaping our service working within the MDT diabetes team and wider hospital specialties so that it is fit for the future for our patients with diabetes while they are residing at the Countess of Chester. You will need to show empathy and sensitivity to the needs of the patients, have good communication skills, together with a friendly, polite, and professional attitude.
You will need excellent clinical, organizational, IT and communication skills.
Work as a highly-skilled independent practitioner implementing and recommending treatment based on expert clinical . Provide comprehensive evidence-based highly specialist clinical care, training, and consultancy to both people with diabetes and other professionals.
Providing advice, support, and education to our patients their families. Contributing to Patient MDT meetings/case conferences, referrals to other services to meet the patients needs .
Assess, advise, and take appropriate action for bleeps, ward referrals emergency and routine phone calls from GPs, practice nurses, district nurses, hospital-based staff and other Healthcare professionals .
Be responsible and accountable for managing the inpatient caseload of patients, with multiple medical conditions and complex needs . To provide an expert diabetes nursing service, giving support, advice, and information diabetes
Act as an expert resource, providing clinical advice and support to others involved in providing care for people with diabetes, in secondary care and patients that need district nurse input in the community.
Work as an independent practitioner and demonstrate specialist skills and knowledge through expert nursing .
Practice advanced clinical skills and maintain a high professional standard of care and expertise'
The Countess of Chester Hospital NHS Foundation Trust provides services to West Cheshire and to Welsh patients covered by Betsi Cadwaladr University Health Board. The Trust works collaboratively within the wider Cheshire and Merseyside Health and Care Partnership. Its services are provided from three locations:
The Countess of Chester Hospital:
providing 438 general and acute beds.
Ellesmere Port Hospital:
providing 56 beds as a rehabilitation, intermediate and outpatient facility.
Tarporley War Memorial Hospital:
a base for community services which serve the local rural population.
The Trust employs over 6258 staff (headcount) which includes temporary bank staff and provides acute emergency and elective services, primary care direct access services and obstetric services to a population of approximately 407,000. This includes 357,000 residents in Chester and West Cheshire which includes Ellesmere Port and Neston as well as the Deeside area of Flintshire which has a population of approximately 50,000.
The Trust is a busy district general hospital and in 2022/2023, there were more than 503,342 patient attendances (inpatient, outpatient and diagnostic) ranging from a simple outpatient appointment to major cancer surgery. This is an increase of nearly 10,000 compared to the previous year when there were 494,387 patient attendances.
