To provide high-quality, person-centred Occupational Therapy services within the HASS Cranbrook and Loxford multidisciplinary team. The post holder will assess, plan, and implement interventions that promote independence, safety, and wellbeing for adults with varying levels of need. This includes assessing complex equipment, recommending major and minor adaptations, and ensuring compliance with relevant legislation and professional standards.
Duties
1. Assessment and Case Management
Undertake holistic Occupational Therapy assessments for adults with physical, sensory, and cognitive impairments.
Identify functional limitations and develop individualised intervention plans.
Manage a caseload of approximately 4 cases per week, covering both complex and standard cases.
Carry out reviews of interventions, ensuring that outcomes are achieved and maintained.
2. Equipment and Adaptations
Assess for, recommend, and provide both standard and complex equipment to promote independence and safety.
Prepare detailed specifications, sketches, and recommendations for major adaptation works.
Liaise with contractors, housing officers, and surveyors to ensure adaptation works meet required standards.
Ensure all recommendations comply with legislation, policy, and best practice.
3. Manual Handling and Risk Management
Conduct manual handling assessments and create safe handling plans.
Provide instruction, advice, and demonstrations to clients, carers, and staff on the correct use of equipment and techniques.
Monitor and review the effectiveness of all prescribed equipment and adaptations.
4. Multi-Disciplinary and Partnership Working
Work collaboratively within a multi-disciplinary team of health and social care professionals.
Participate in joint assessments, duty systems, and case discussions.
Contribute to integrated care planning, ensuring a seamless service for service users.
5. Compliance, Recording and Legislation
Maintain accurate, up-to-date records and reports in line with professional and organisational standards.
Ensure practice is compliant with the Care Act 2014, Housing legislation, and other relevant frameworks.
Follow HCPC and RCOT standards of proficiency, maintaining professional registration and ethical practice.
6. Professional Development and Supervision
Participate in supervision, training, and continuous professional development to maintain and enhance knowledge and skills.
Reflect on and evaluate practice to support service improvement.
Support the development of good practice within the team and contribute to the delivery of service objectives.

Experience
Professional qualification in Occupational Therapy recognised by the Health and Care Professions Council (HCPC).
Excellent knowledge of specialist equipment, manual handling, and major adaptations.
Experience working in a multi-disciplinary environment.
Understanding of social care and housing legislation and their impact on practice.
Ability to plan, prioritise, and manage a complex caseload independently.
Experience providing both simple and complex equipment recommendations.
Current HCPC registration.
