This is a part-time, permanent role with hybrid working, and a salary of 24, 895 - 28,132 pro rata per annum.

The Role:



The

Occupational Therapy Assistant

role is within the Integrated care team/Locality Access Team (LAP) which consists of Community Matrons, Mental Health Practitioners, Social Workers, Occupational Therapists, and Social Care Practitioners.

The LAP team involves supporting people with multidisciplinary support to look at the needs of the resident in a holistic way helping to prevent longer term challenges and admissions to hospital.

Your role will involve:



Assessments and interventions for residents with physical disabilities, not excluding cognitive decline Working with other professionals to support identified outcomes Holding and managing a case load
