Whittington Health Adult Community Services are recruiting and looking for an individual with enthusiasm, drive and a passion for Parkinson's disease, to support the well-established Community Parkinson Disease service.



The service is small and consists of one Parkinson Disease Specialist Nurse Consultant, who covers across acute and community settings.



The successful candidate will act as a highly skilled resource within the field of Parkinson's Disease (PD). To deliver safe and effective interventions, advice, and support for people with Parkinson's disease and their families.

To manage a caseload of patients with PD through the four stages of the illness from diagnosis, maintenance, complex and palliative, using evidence-based principles to assess individual needs, set and implement programmes of care, evaluate interventions in both the hospital and community setting.

The postholder will provide specialist advice, support, and education for health professionals in both primary and secondary care settings.



Will participate in planning, developing and evaluation of PD services within the London Borough of Haringey and Islington.



Will contribute to research-based nursing practice and monitor the quality of the service provided. Undertake audit, CPD, teaching, research, and administration.



The postholder will deliver education and training regarding the management of Parkinson's to patients, families, health, and social care professionals in both primary and secondary care, including private and voluntary organisations.



Will be required to act as a highly skilled resource within the field of Parkinson's Disease and provide specialist advice, support, and education for professionals in both primary and secondary care settings. To deliver safe and effective interventions, advice, and support for people with Parkinson's disease and their families/carers.

The postholder will be required to deliver an education package regarding evidence-based practice of Parkinson's to all health and social are care professionals.

T o work in partnership with people who are affected by Parkinson's Disease to develop a primary care led Parkinson's Disease service which places the patient at the centre.

To provide clinical care to people with Parkinson's Disease. This will include patient and carers advice and support, facilitation and advice on access to therapy and other support services, monitoring and adjustment of medication.



The post holder will also be involved in formulation and monitoring of standards of clinical care, promotion of the availability of relevant services for clients and ensure that all clinical practice is evidence based, adhering to national guidelines relating to PD and Parkinson's Plus syndromes including audit of outcomes.



To develop and establish entry and exit points on integrated care pathways, self-referral mechanisms for patients to access services and explore methods to improve information, communication and record systems.
