The Role


We are recruiting a Patient Safety Investigator to join our Clinical Governance team, supporting the continuous improvement of patient safety across The London Clinic. This role plays a vital part in our Patient Safety Incident Response Framework (PSIRF) approach, ensuring that incidents are investigated thoroughly, fairly and transparently, and that meaningful learning is embedded into practice.
You will work closely with clinical and non-clinical teams, guiding and supporting staff through the investigation process, analysing data and evidence, and helping to deliver improvements that enhance the safety and quality of care. You will also liaise with patients, regulatory bodies and insurers, representing The London Clinic's open and learning culture.
The successful applicant will need to have a clinical background and experience or demonstrable knowledge of the Patient Safety Incident Response Framework (PSIRF), and an analytical mindset. Previous experience of working with Datix is desirable.

Key Duties:

Lead and support patient safety investigations to ensure timely, proportionate reviews that drive organisational learning. Conduct investigations using systems-based and human factors methodologies to identify root causes and actionable recommendations. Gather and analyse evidence, including clinical records and staff interviews, to ensure clarity and accuracy in reporting. Produce high-quality investigation reports that highlight key findings and learning outcomes. Analyse Datix data to identify patterns, trends and themes that can inform improvement initiatives. Collaborate with clinical teams to track, implement and review improvement actions. Assist with patient safety training and workshops to raise awareness of PSIRF and best practice. Engage with patients, insurers and external regulators to support open communication and ensure compliance.

This role is an excellent opportunity for a nurse or clinician who is looking to move away from direct patient care but still wishes to use their clinical expertise to make a meaningful impact.
You may currently be working in a ward, theatre, or outpatient setting and are ready to transition into a role focused on quality, safety, and improvement. You will enjoy examining how and why things happen, finding practical ways to prevent recurrence, and supporting colleagues to deliver safer care.
You will have a natural curiosity, attention to detail and confidence to work across departments, alongside the ability to communicate sensitively with patients and staff. If you are passionate about learning, improvement and influencing positive change across an organisation, this role will suit you well.

Skills and Experience:

Registered clinician with a UK professional body. Awareness of clinical governance, the patient safety framework and CQC regulatory compliance. Knowledge of PSIRF, and ideally PSII training or experience. Working knowledge of Datix Web is desirable. Proven experience in clinical governance, patient safety or quality improvement within a healthcare setting. Relevant clinical experience in a hospital environment. Experience in conducting or assisting with investigations into safety incidents. Strong analytical and organisational skills with the ability to interpret data and identify trends. Skilled in report writing for assurance and producing evidence-based recommendations. Demonstrated ability to manage patient interactions with empathy, compassion and professionalism. Competent user of Microsoft Word, Excel and PowerPoint.
