Job Description

The Rehabilitation Health & Care Support Worker delivers delegated care/tasks across a range of disciplines as part of a team-based approach to delivering holistic patient centred care.



A key aim of the role will be to support the patient to achieve optimum independence by assisting in the provision and monitoring of rehabilitation services, implementing an agreed individualised plan of care as determined by the team.



The post holder is expected to always treat individuals with dignity and respect, recognising their wishes and beliefs empowering the patient to have choice and control of their care.



The post-holder will practice in accordance with Health Board standards and statutory requirements and operate within the boundaries of the role and assessed competencies.



The post-holder will be expected to develop within the role as required within the boundaries of the job description and agreed competencies.



The post holder must communicate effectively at all times and in conjunction with other team members contributing to and maintaining the team/ward philosophy.



The successful applicant will:

designated by the registered professional, undertake identified tasks within the framework of a plan of care. be confident & competent to work without continual direct supervision within an agreed framework, assist patients/clients to achieve physical and emotional well-being and comfort, including assessments of clinical condition as required, supporting optimum independence, within scope of competence. have the ability to identify and support the needs of a wide spectrum of conditions, including noticing issues and appropriately acting upon signs of frailty, including malnutrition, difficulties with eating and drinking and reduced mobility. identify and report the social network/support and care infrastructure of clients i.e. carers, family, already known to partner agencies. ensure that any changes in patient's condition or circumstance are reported immediately to the registered professional as appropriate to the needs of the patient.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire

Two integrated care centres:

Aberaeron and Cardigan in Ceredigion, and several other community settings

47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers
