/ Senior Outreach Worker (Overseeing 3-4 Stepdown Homes)



Hours:

45 hours per week, including alternative weekends

Minimum Level 3 Diploma in Working with Children and Young People (or equivalent)

Good IT skills and strong report writing skills

Role Overview



Provide structured, outcome-focused keywork support to young people aged

18-25 leaving care

with diverse and complex needs, across

3-4 stepdown homes

. Support includes developing independence, resilience, and confidence, helping young people transition successfully into adulthood.

Young People's Backgrounds May Include:



Experience in foster care, residential care, or supported housing Histories of neglect, abuse, or disrupted family relationships Mental health, emotional wellbeing, or behavioural challenges Educational gaps or limited engagement Needs for independent living skills support, including budgeting, cooking, and tenancy management Aspirations for education, training, employment, or personal development

Key Responsibilities



Key Working & Welfare Support



Provide structured support tailored to each young person's needs. Monitor wellbeing, safety, and engagement across assigned homes. Implement placement plans and advocate on behalf of young people. Liaise with social workers, PAs, healthcare providers, colleges, and other professionals. Document work: contact notes, weekly sessions, monthly reports, and incident reports.

Support young people with tenancy agreements, repairs, moving into their own flats, and encourage independence with ongoing support.

Assist with housing applications, Universal Credit, and registration with local services. Support attendance at health, legal, education, employment, and housing appointments.

Welfare Checks & Safety



Conduct welfare checks if no contact within 48 hours and follow escalation procedures, including contacting NOK, hospital, social workers, or police if required.

Risk Assessment & Support Plans



Develop, complete, and review risk assessments, support plans, and behaviour plan as needed. Submit all plans to the registered manager for review and approval.

Room & Communal Area Responsibilities



Conduct weekly room checks, supporting young people to maintain cleanliness. Maintain clean and tidy communal areas, bathrooms, kitchens, lounges, and gardens. Encourage young people to participate in cleaning rosters.

Health & Safety Compliance



Maintain the Health and Safety Compliance Folder on SharePoint. Conduct

monthly health and safety checks

and ensure all certificates are up to date. Perform

monthly carbon monoxide (CO) alarm checks

for each home. Report any concerns immediately to the registered manager. Conduct weekly room and communal area checks for repairs and safety issues. Conduct fire safety inductions for new young people. Carry out weekly fire alarm tests and monthly checks of first aid boxes and smoke alarms.
Job Types: Full-time, Zero hours contract

Pay: 15.00 per hour

Expected hours: 45 per week
