About Alliance for Better Care CIC



Alliance for Better Care (ABC) is a GP Federation uniting 77 NHS GP member practices across 98 sites within 24 Primary Care Networks in Sussex and Surrey. We support our Primary Care colleagues - and their patients - to transform how healthcare is delivered in their communities.

We work closely with GP Practices, PCNs, Hospitals, Community Organisations, and the Third Sector. These vital partnerships enable us to deliver a truly integrated approach that offers the support and expertise needed to effectively serve our populations.

Our Equity Team?delivers clinical outreach services to people who experience barriers to healthcare, including those facing homelessness, refugees and asylum seekers, Gypsy, Roma and Traveller communities, and other groups experiencing health inequalities.

Our?Refugee and Asylum Seeker Service (RASS)?provides trauma-informed healthcare in hotels across Surrey and Sussex. The clinical team includes experienced GPs, nurses and paramedics who provide on-site urgent care, health checks, vaccinations, wound care and women's health sessions. The team works closely with local primary care and public health partners to ensure continuity of care and improved access to NHS services for people who have newly arrived in the UK.

Job Summary



The post-holder will form part of ABC's RASS and Equity clinical team, delivering outreach healthcare to communities experiencing barriers to accessing mainstream services.

They will support the delivery of?health checks, smoking cessation support, vaccinations and MECC-based health promotion?in both community and hotel settings used for?Refugee and Asylum Seeker accommodation.

Working under the supervision of the Lead Nurse and following agreed clinical protocols, the post holder will:

Provide opportunistic and planned health promotion interventions. Deliver smoking cessation support and education. Assist in the delivery of vaccination clinics and pop-up health checks. Support individuals to access and engage with primary care. Contribute to ABC's wider inclusion and prevention work through collaborative, person-centred care.
You may be required to undertake all or some of these duties depending on service needs. Full training and supervision will be provided.

Primary Duties and Areas of Responsibility



Clinical Support

Carry out basic health checks (blood pressure, BMI, pulse, lifestyle screening) and document findings accurately. Support pop-up vaccination and health events--preparing and restocking areas, maintaining cold chain procedures, and assisting clinicians. Undertake phlebotomy, ECG recording, and wound care if trained and competent. Support the delivery of smoking cessation clinics, including assessment, counselling, and follow-up support. Apply the?Make Every Contact Count (MECC)?approach, using every patient interaction to promote healthy lifestyle changes. Assist in opportunistic screening and support with health promotion conversations during outreach visits.
Health Promotion & Smoking Cessation

Provide one-to-one and group-based smoking cessation advice and behavioural support. Educate patients on the risks of smoking, available treatments, and coping strategies. Support patients with nicotine replacement or other cessation therapies under clinical guidance. Promote wider health awareness--covering vaccination, screening, mental wellbeing, and self-care--through engagement at community events and outreach sites. Work as part of the Equity and RASS clinical team to identify and engage individuals from inclusion health cohorts. Support people to register with GP practices and access community or secondary care where needed. Deliver care that is trauma-informed, culturally sensitive, and person-centred. Liaise with interpreters, support workers, and voluntary sector partners to ensure clear communication and holistic care. Escalate any safeguarding concerns in line with ABC and statutory safeguarding procedures.
Administration & Governance

Maintain accurate, confidential patient records (manual and electronic). Contribute to data collection and reporting for service monitoring and evaluation. Follow ABC's policies on infection control, confidentiality, data protection, and safeguarding. Attend supervision, mandatory training, and team meetings to maintain competence and reflective practice.
Supervision

Regular supervisions on a one-to-one basis including an annual formal appraisal will be held with a line manager. If you are in a patient-facing role, you will also receive regular Clinical Supervisions with a Clinical Supervisor.

Working hours



This role is available as one full day per week or two half days.

Qualifications

Care Certificate or NVQ Level 3 in Health and Social Care - essential Smoking Cessation Advisor training or equivalent; Phlebotomy or ECG certification - desirable
Experience

Experience working with vulnerable or inclusion health groups - essential Experience in community or primary care settings - essential Understanding of safeguarding adults and children - essential Experience delivering smoking cessation or MECC-based interventions - desirable Experience in outreach or health equity programmes - desirable

Pay: 13.69-14.81 per hour
