Brief Description:

Somerset Activity and Sports Partnership (SASP) is seeking an Activator to deliver our Move 2 Independence (M2I) reconditioning service across Somerset. M2I supports adults to live well again at home following a stay in hospital with Activators providing one to one movement and coaching sessions to guide and empower movement for independence, resilience and preventing re-admissions. An Activator is required to support M2I across Somerset. The Activator will receive a case load of participants referred from Somerset hospitals and bring in people's support networks, where appropriate, to help build confidence and capability for longer term physical activity. Activators require independent means of transport and be able to work in people's homes and local communities to provide a truly person-centered service.

Personal Specification:

We are looking for a compassionate, self-motivated individual with a genuine passion for supporting others to regain independence and improve their wellbeing. The ideal candidate will have experience in delivering person-centred physical activity or coaching interventions, preferably within health, care, or community settings. Strong communication and interpersonal skills are essential, along with the ability to work independently and flexibly across Somerset. You must be confident working in people's homes and local communities, and able to build trusting relationships with participants and their support networks. A full driving licence and access to independent transport is required.

Owing to the nature of this position, any offer of employment will be subject to a satisfactory Enhanced DBS check.
