The Activity Coordinator assesses, plans, implements and evaluates programs for individuals' activities which are multi-faced, meet the patient's functional needs, and must reflect
interests of each patient in developing a stimulating activities programme that will meet our residents emotional, social, physical, and intellectual; needs to enhance the quality of life for everyone within the care home.
You will need organisational skills and a can-do attitude to make things happen. We welcome someone who is brimming with ideas and has an 'anything is possible' approach to all they do with an infectious enthusiasm and creativity to inspire our residents and teams to get involved with activities within the home and the local community.
1. Create, implement and deliver a consistent, sustainable and inclusive programme of activities linked to residents' interests as well as their emotional and physical wellbeing.
2. Actively research local resources and support community engagement and create links with outside organisations to promote and facilitate local amenities and community.
3. Carry out assessments of individual likes, dislikes, interests, and skills and complete "This is me" with residents and their support systems to ensure meaningful activities provide that enhance wellbeing and skill development.
4. Plan and organise an activities programme that will maintain interest and progression over a period identify the needs of the collective group and individuals and tailor the activities to suit them.
5. Assist Residents in all aspects of their daily activity programme.
6. Pay particular attention to assisting Residents who have limited mobility or physical / learning difficulties, making the best use of aids provided.
7. Provides residents who are confined or choose to remain in their rooms with in-room activities in keeping with life-long interests (music, reading, etc) and in-room projects they can work on individually.
8. Support and develop Residents who may be confused and / or who have behavioural problems.
9. Observe care planning needs for Residents and complete written daily records as instructed and in line with the Company's policies and procedures.
10. Under the supervision and guidance of RGN, report on the well-being of Residents.
11. Ensure full privacy and dignity is maintained in line with the Company's policies and procedures.
12. Assist in planning and implementing themed lunches, afternoon teas and dinners.
13. Documents activity related information on the Residents care plan.
14. Maintains sufficient activity supplies.
15. Develop monthly calendar of planned activities to coincide with local, national, and worldwide events.
16. Encourage involvement in the local community.
17. Plan and organise external outings with appropriate transport.
