Part time - 25.5 hours per week



An exciting opportunity has arisen to join the Acute Care Response Team at Great Western Hospital in Swindon. The successful post holder will be responsible for overseeing the high standards of care required for looking after the deteriorating / critically ill patients on the ward and following up those patients who have been discharged from Critical Care.



As a skilled member of the ACRT, you will provide intervention with appropriate assessment, observations and treatment for seriously ill patients on the general ward. To be an expert clinical practitioner and provide clinical advice and support to ward based staff to enhance their knowledge and skills in caring for seriously ill patients in the ward areas.



The post holder will be expected to provide professional leadership, advice and guidance for nursing practice within the hospital and being a resource for all nursing staff.



They will also need to have expert clinical skills, great communication skills and clinical assessment skills. They will contribute to ensuring that the nurses within the hospital are developed to maximize their contribution to the achievement of the Trusts service goals and upholds and models the Trust values.



To be an expert practitioner in nursing acutely ill patients. Act as a professional role model who leads by example in clinical skills, attitude, conduct and appearance.



Develop clinical skills in the assessment of seriously ill patients on general wards.



Clinical confidence and competence to manage haemodynamic instability and provide appropriate respiratory and vital organ support within the ward setting with minimal guidance.



Participate in inter-hospitals transportation of patients.



Participate in the development of strong links between members of the multidisciplinary team to enable swift and appropriate identification and management of critical care patients wherever their location.



Undertake full assessments and reviews of patients on general wards, including those patients who have been referred to the ACRT and those who have recently been discharged from intensive care / High Dependency, liaising with the ward team and Critical Care team where appropriate.



Facilitate the seamless transfer of critical care patients between different levels of care, liaising with appropriate clinicians to ensure patients are nursed in an appropriately skilled, safe environment according to their clinical need.



Follow up post intensive care / High Dependency patients and respond appropriately to highlighted issues, providing both physiological and psychological support.

Contribute to the delivery of formal and informal teaching at all levels of the multi-disciplinary team.



In conjunction with the Nurse consultant identify and respond to educational issues in the ward areas.



To utilise and share research findings and audit recommendations in the delivery of practice which is evidence based and current best practice.



Support and motivate staff in the provision of care, imparting knowledge and skills whilst working with nurses caring for patients in general ward areas.



Develop inter-professional collaboration, and work in partnership with ward-based staff across the Trust.



Attend meetings and act as a representative of outreach throughout the Trust.



Participate in the development, implementation and review of standards, policies, procedures and clinical guidelines related to outreach issues across the Trust.



Participate in data collection for research and audits relevant to post.
