The Link Charitable Trust (LCT) is a Redcar based charity working with children, young people, families and vulnerable adults. We support individuals to make positive changes to their lives through a range of emotional, social and wellbeing activities and services within the community. We offer open access to support, advice and guidance, for as long as it is felt to be needed.
Job Purpose
The post holder will be working within The Link Charitable Trusts Adult service provision, overseeing the overall project and linking together with other projects within the service. We are looking for an experienced Project Coordinator who is passionate about promoting positive mental health, and who can support the growth of the provision we offer to adults in the community. The candidate will also have experience of supporting adults, as part of the role will be to support group delivery and offer 1:1 assessments/support.
Main Duties and Responsibilities
- Development, coordination and management of the Adult Mental Health Service.
- To be the initial point of contact for Adult Safeguarding issues within the Link Charitable Trust.
- Conduct Operational case management supervision with Adult Mental Health workers and other project workers, ensuring the service user is receiving the best service possible, adhering to time frames and copy output policies.
- Carry out line management supervision of the Adult Team.
- To screen referrals that come into the service and monitor the waiting list.
- To be responsible for overseeing the assessment of all new service users, and ensuring the team carry out competent initial assessments.
- To allocate new service users to the project workers, counsellors and volunteers, ensuring that their caseload is manageable and continually ensuring work output is in line with current policies.
- Ensure accurate and timely input of case notes and outcomes onto the database.
- Continuous review and development of appropriate Outcomes and Evaluation Tools for use throughout the project.
- Oversight of volunteer recruitment and supervision to enhance the project and the service user journey.
- Contribute to and have a comprehensive understanding of the project budget and project deadlines, ensuring KPI's are met.
- Ensure up to date data collection and statistics to inform project feedback.
- Ensure reporting deadlines are adhered to, and systems are in place to accurately capture service data for reporting.
- To support the Charity Manager in seeking opportunities for grant funding, and to contribute to funding applications.
- Map out and forge links with other voluntary sector networks and organisations working with people who may require additional support due to their challenging life circumstances.
- Research similar services and opportunities operating locally and nationally to source advice and guidance on potential pitfalls and successes.
- Ensure that the service remains 'fit for purpose' to meet the needs of families using the project listening to new ideas and seeking to meet any new areas of need identified.
- To deal with general enquiries from the team, including bringing them to the attention of appropriate internal/external services in a timely manner.
- Work in line with The Link Charitable Trust Policy and Procedure, including, Health and Safety, Equality and Diversity and Data Protection.
- Act at all times in the best interest of the service users, recognising potential vulnerability in line with Safeguarding procedures.
- Required to undertake other duties commensurate with the role as required by business need, not specified in the job description.
- Undertake all work in accordance with service policies and procedures.
- Engage in and respond to personal development supervision to improve competencies and practice.
Direct Work with Adults
- Provide assessment, including risk assessment of parents/ carers and other adults experiencing mild to moderate mental health difficulties.
- You will oversee, and support the facilitation of a range of wellbeing groups.
- Build relationships with adults who are sometimes reluctant to attend mental health services, who are seen as 'hard to reach'.
- Educate and involve family members and others in support as necessary.
- Attend multi-disciplinary meetings relating to referrals or clients in treatment where appropriate.
- Keep coherent records of all activity in line with service protocols.
- Work closely with other members of the team ensuring appropriate step-up and step-down arrangements are in place to maintain a stepped care approach.
- Assess and integrate issues surrounding work and employment into the overall therapy process.
Supervision
- Receive clinical supervision monthly in relation to clinical work to meet the required standards (if this is applicable to the level of support you are trained to deliver).
- Receive management supervision monthly to discuss project outputs.
- Prepare and present case load information to supervisors and case managers within the service on an agreed and scheduled basis, in order to ensure safe practice and the governance obligations of the trainee, supervisor and service are delivered.
- Respond to and implement supervision suggestions by supervisors in practice.
- Work within the boundaries of service delivery and support to meet the requirements of the service needs.
- Engage in and respond to personal development supervision to improve competencies and practice.
Monitoring, Evaluating and Developing Service Provision
- Assist the service and colleagues in the monitoring, evaluation and development of the service with the tools and methodology provided for such.
- As appropriate, contribute to the development and learning of other service providers with regards to improving outcomes for children, young people and families within the service.
- Contribute to the setting of targets/action plans and work related to these, to assist service development.
- As appropriate, assist in the development and production of resources for the benefit of children, young people and families.
- Support children, young people and families to be involved as much as appropriate (according to age and understanding), with regards to involvement in the monitoring, evaluation and development of the service.
- In the absence of the line manager, assist in ensuring service delivery and development is maintained to a good and professional standard at all times.
Administration and Finance
- Contribute to the day to day administration and running of the project, including appropriate keeping of all records relating to the users of the service, administration and finance, security and confidentiality of information/communication.
- Share appropriate information with children, young people and families, staff and volunteers and other agencies in accordance with the rules of confidentiality and protection of data.
- Ensure the budget for the project is adhered to and if necessary alert management to any potential overspends which may occur.
Work with Other Staff/Agencies
- To create and maintain effective working relationships with external agencies.
- To liaise effectively with other local providers, on behalf of the service user.
- Present the users of our service and the service overall in a positive and professional manner, not bringing the service into disrepute.
- As appropriate, represent the service and provide a positive service perspective at relevant inter-agency meetings (or other forms of joint working or training).
- Liaise with other staff and agencies to further meet the needs of service users in accordance with the service criteria, policies and procedures.
- Take part in the delivery of training (as appropriate).
A Professional and Ethical Service
- To maintain appropriate notes and records on Lamplight and in line with company Policies and Procedures.
- To undertake relevant Continuing Professional Development as required for the purposes of continued professional accreditation or registration and as identified through The LCT's professional development and Appraisal system.
- To undertake clinical supervision and case management as per The LCT's supervision policy and in line with the appropriate professional body.
- To work within The LCT's GDPR and Information Governance policies and procedures.
- To ensure safe and secure working environment for self and others and to take action as necessary within Health and Safety and other guidelines.
- To adhere to all company Policies, Procedures and Guidelines.
- To promote and value the rights, responsibilities and diversity of people using the service.
- Promote and act in accordance with the service action plans with regards to equal opportunity and non-discrimination.
- Undertake training, supervision and appraisal to provide a high standard of service in a learning, innovative and continuous personal development environment.
- To promote the safety of children, young people and families to further the aim of preventing abuse or the risk of abuse.
- To act in a professional manner at all times whilst representing the service.
- Undertake other duties and responsibilities as required from time to time, commensurate with the post.
