Job overview
The Community Neuro Services offers services to patients with Stroke (including Early Supported Discharge (ESD) response and other Long term Neuro conditions. The ESD response pathway provides a flexible and proactive therapy service to facilitate early discharge of patients from acute, inpatient settings for clients with a new stroke
diagnosis who have ongoing therapy needs.
The Long term conditions pathway provides a specialised multi-disciplinary service to patients with neurological conditions in the community in order to safeguard independence by preventing inappropriate hospital stays, maximising rehabilitation and recovery after illness or injury, and minimising premature dependence on long term institutional care.
They are responsible for the provision of a comprehensive community Neuro service to clients with a neurological condition who have multi-disciplinary needs and have a GP within Sutton ICB.
Ensuring the needs of the patients are identified and met. Promoting the
independence of patients, assisting them to maintain/or improve their own physical, psychological and social well-being.
Therapists in the team assess, differentially diagnose, plan & implement rehab/ intervention programmes and manage a caseload of adults with neurological conditions in the community setting.
Educating carers, family members, other professionals, supervising students and mentoring junior staff, are also a requirement of the role. Therapists are expected to work flexibility across the Neuro service.

Clinical
To work as part of a multi-disciplinary team of therapists and rehabilitation assistants to provide specialist neurological intervention to enable and rehabilitate patients registered with GP's within Sutton CCG.
To provide rehabilitation to patients with a range of neurological diagnoses who meet the Community Neuro Services criteria. To have clinical responsibility for assessment and treatment planning of a caseload of clients within the community, including those based in residential and nursing home facilities.
To develop a high level of clinical skill in community rehabilitation and longer term management of disability and impairment.
To provide comprehensive occupational therapy intervention to clients in the community, making use of treatment facilities where appropriate.
To be responsible for a clinical caseload working within a multi-disciplinary team ensuring clients and carers receive optimum level of therapy within available resources.
To carry out assessments, plan and implement agreed goal orientated occupational therapy intervention with client/carers in an appropriate community setting.
To carry out multi-disciplinary screening assessment and discuss outcomes with team members.
