We have an exciting opportunity within the Mental Health Social Work Team.

This 12-month fixed term contract could be filled by either:A full-time Adult Practitioner, or A 0.8 WTE social worker.


This role presents an exciting opportunity for a professional who is passionate about mental health and committed to supporting young people through transitional care through Young People Calm Cafe. The service is prevention-focused, community-based mental health support initiative designed to reduce social isolation, prevent mental health crises, and promote recovery and resilience. The post holder will be responsible for the smooth operation of the Young People Calm Cafe, working in partnership with A Better Tomorrow Voluntary sector to provide early intervention, planned day activities, support, signposting, etc as required in a non-stigmatising and welcoming environment.

Mental Health Team

: We are a small, dynamic, diverse and dedicated team that covers the whole of the Telford Borough. We are passionate in supporting people in their recovery journey to positive mental health and wellbeing. With an inclusive focus, incorporating their families, carers, and communities, to enable the person to live fulfilling, and an independent life. We work closely with the voluntary sector, Community Mental Health Services, Mental Health hospitals, Forensic services, housing and other relevant services in the community to support adults during their mental health journey. As a team, we have a presence in the community in the form of calm cafes, which we run jointly with Telford MIND and A Better Tomorrow to support people who are experiencing mental health crisis and comorbid addiction challenges. The team has a strong connection with the Mental Health Alliance which is a collaboration of voluntary sector, statutory and NHS services adopting a Multi-disciplinary Approach which allows sharing of information to support individuals and communities to promote positive outcomes.

Key responsibilities:Co-ordinate Calm Cafe sessions in partnership with a voluntary organisation, ensuring an inclusive, welcoming and safe environment. Ensure the Cafe supports young people transitioning from children's services, helping them build independence and essential life skills. Promote the Calm Cafe as a preventative space that encourages empowerment, peer support, social connection and wellbeing. To provide 1:1 interventions to support young people attending calm cafe at a time of having a crisis. Work to reduce stigma associated with statutory services by offering accessible and informal support pathways through the Calm Cafe model. Collaborate with community, voluntary and statutory partners to continually develop the offer and reach of the Cafe. To undertake community development work to link with existing and establishing provisions for young people in the community. Provide person-centred support to attendees, including brief interventions, mental health advice, and crisis de-escalation. Complete Care Act 2014 assessments, Carers Assessments and Section 117 aftercare reviews, as required. Offer information, advice and signposting to relevant services and community resources to promote self-management and resilience. Apply a strengths-based, proportionate approach to risk assessment and positive risk-taking. Build strong working relationships with community organisations, voluntary partners and statutory services. Encourage peer-led support and co-production with people who have lived experience. Contribute to service evaluations, audits and data collection to demonstrate outcomes and inform future development.
