About the role

The South Learning Disability Partnership offers an exciting opportunity for a Adult Support Coordinator. This 37-hour-per-week role combines office-based work at Sunley House in Papworth Everard with home working.


Adult support coordinators play a major part in the work we do in our team, and you will be working with adults with a learning disability, as well as their unpaid and formal carers and if successful, you will be working alongside social workers as well as other health professionals including Community Nurses, Occupational Therapists, Art / Music Therapists, Speech and Language Therapists, Psychiatrists, Psychologists, and Physiotherapists to meet the outcomes of those we support.

What will you be doing?

Join our team of dedicated and highly skilled professionals working on cases within community and institutional settings. You'll work with adults in need of care and support services, along with their unpaid carers, partnering with them to establish and meet their needs.


As an Adult Support Coordinator, you will complete assessments, support planning, mental capacity assessments, and carers conversations, continually seeking innovative ways to improve care delivery for the benefit of service users.


This role focuses on providing service users with choice and control over their lives, empowering their independence.


About you

We are looking for an energetic and motivated individual who enjoys making a difference and building relationships. You will demonstrate excellent written and verbal communication skills and work both independently and as part of a multi-disciplinary team. You will commit to making a positive difference for the people we support. The successful candidate will have excellent organisational skills, use their initiative to work innovatively, and work from various locations and travel countywide independently.


You don't need a social work qualification, but knowledge, experience, and understanding of the needs of vulnerable adults are essential for success in this role. Working knowledge and experience of the Care Act, Mental Capacity Act, and Cambridgeshire County Council's Adult Social Care policies and procedures will also be advantageous.
