About the role

Our Transfer of Care Team is dedicated to managing patient discharges from acute and community hospitals across Cambridgeshire. Please note that the main base for this role will be covering Addenbrookes Hospital but there is an expectation to cover the other hospitals within Cambridgeshire and undertake community based work when required. We pride ourselves on a Home First approach, ensuring individuals can stay within their own communities while receiving the care and support they need.


We do work weekends, but our staff receive a salary enhancement between 7% and 15% depending on their selected working pattern.

7% for 1 weekend day a month and 1 Bank Holiday a year 8% for 1 weekend day a month and 2 Bank Holidays a year 10% for 1 weekend day a month and 3 Bank Holidays a year 15% for 2 weekend days a month and 3 Bank Holidays a year

What will you be doing?

You'll be working with individuals and their informal carers to complete Care Act assessments, care and support plans, carers assessments and initial reviews alongside having a role in Safeguarding and considering capacity.


Inquisitive and creative practice is embraced, to allow for personalised solutions, taking into account wider community resources and organisations.


The work will be ensuring timely discharges from hospital as well as working closely with system partners such as Reablement, Cambridgeshire and Peterborough Foundation Trust (CPFT) and the Cambridgeshire and Peterborough Integrated care system (ICB).


This role involves working closely with social care and health colleagues to ensure correct discharge plans are in place.


About you

You will need to have good and accurate literacy, numeracy and administrative skills and the ability to analyse information to solve general queries. It is imperative that you have strong communication skills, a confident telephone manner and can work on your own initiative. Good planning and organisation skills will be key, and you should be used to working under pressure to meet deadlines, allowing flexibility and resilience. You will need a good working knowledge of the Care Act 2014 and Mental Capacity Act 2005 including experience of multi-disciplinary working to enable best outcomes for individuals.
