1 x Grade 6 Support Worker to our Adult Transitions Team.

This post is a temporary 18-months vacancy.

The Adult Transition team and the Autism service are two teams that work closely together to support young people 18-25 with a range of disabilities. We work closely with colleagues from Children's services, including social care, Education and Health to plan for young people approaching adulthood, we begin getting to know young people from when they are 14 so that we can plan well with them and their families / carers to ensure they have a smooth path into adulthood and beyond. The team consists of social workers, social care assessors, Support worker and an Occupational therapist. We are a strong and supportive team who are passionate about the work we do.

The job is about supporting people:


Preparing young people and their families for successful and fulfilling adulthoods Listening to the young person however they may communicate to ensure their voice is at the centre of planning With dignity and care to maximise their independence, putting them at the centre of everything you do Working closely with families to support them to promote the independence of their young person Supporting to build skills Accessing the community, shopping, clubs, groups, swimming, socialising with family and friends joining in too Keep accurate and up-to-date records and contribute to and follow care plans. Ensure they are safeguarded at all times. And work flexibly as part of a team

We are looking for someone who:


Shares our values Has a commitment to people with Autism, learning and other disabilities Has the ability to learn and develop skills and a commitment to gaining level 2 QCF/NVQ in Social Care (or who already has it) Able to support people in the community e.g. shopping, swimming, clubs and activities Reliable, flexible and a team player
