Pay rate - 32.52 per hour
Contract role, Frailty and Anticipatory Care Team
Pertemps are recruiting for experienced Adult's Social Worker in North London.
This is to work in a Frailty and Anticipatory Care Team
Key Responsibilities
- Deliver strengths-based assessments and interventions under the Care Act, Mental Capacity Act, and other relevant legislation.
- Work collaboratively with residents, carers, community organisations, and health professionals. o Attend and contribute to multi-disciplinary team (MDT) meetings,
- Identify and respond to safeguarding concerns using a Making Safeguarding Personal approach.
- Maintain accurate records using the LAS electronic system, ensuring timely documentation of outcomes and progress.
- Support anticipatory care planning and early intervention to prevent escalation of need.
- Promote the values of CARE (Collaborative, Ambitious, Resourceful, Empowering) and iCARE (Innovation, Compassion, Accountability, Respect, Excellence).
