Sefton's leading care provider, New Directions, is now recruiting for an Advanced Care Coordinator. We currently employ around 350 staff across the business who pride themselves on delivering high quality social care and support to vulnerable groups within the Sefton area.
Lyra Road offers a personalised service to adults who have learning disabilities and/or autism. Our staff work closely with individuals to develop their knowledge and understanding, highlighting key community values of respect, choice, dignity, privacy and stability.
Our Supported Living teams are on hand to provide support with relationships, nurture opportunities to develop new skills and advise people on their finances, all of which enables individuals to live as independently as possible.
The successful candidate will oversee the delivery of a range of services in the Supported Living Scheme for people with learning disabilities. To work with individuals in a variety of settings to promote greater choice, skill development, independence and inclusion.
There is a requirement to work evenings, weekends and bank holidays in accordance with a planned rota. Sleep in duties are also required for which you will receive an additional payment.
Experience of working with vulnerable people in residential based settings is essential. A Level 2 Diploma in Health & Social Care or equivalent is also essential.
An Enhanced DBS Check is required.
