Advanced Care Practitioner
Job Description
The post holder will place the well-being of safety of service user / patients above all other consideration whilst ensuring that they are working within the limits of their professional competence Autonomously prescribe medications within agreed local and national protocols to patients based on clinical assessment and diagnostic reasoning. Patients will be holistically assessed to determine a differential diagnosis and identifying further specialist assessment that maybe required To continually assess the service and ensure that processes for capturing data are in place to be able to evidence key performance indicators. Provide supervision and personal development support to junior staff. Undertake investigations ensuring any recommendations are embedded into practice.
Ensure that documentations are in line with Trust standards. Work across professional and organisational boundaries to share good practice and provide / support educational programmes Oversee the building and monitor use of the estate.
