The post is in the area of North West Surrey working at Sunny Meed Surgery within the WISE 2 Primary Care Network. It offers the opportunity to further develop and utilize clinical knowledge and prescribing skills.

You

will be motivated, and passionate about patient care, and have excellent communication skills to work with the multi-disciplinary healthcare team across the locality to optimise medications with a patient-centred approach. With the increasing burden of polypharmacy and complex co-morbidities in the elderly population, we are looking for a pharmacist with a keen interest in chronic disease management, with understanding of balancing risks and benefits in relation to medicines and patient circumstances. This advanced pharmacist post is a great opportunity for pharmacists wishing to further or develop their career.

Successful candidate will need to have substantial post qualification experience, and be willing to undertake the CPPE 18 month pathway followed by the independent prescribing qualification if not already completed.

The

main parts of this role

is supporting the duty doctor with day-to-day queries, medicines reconciliation, undertaking chronic disease reviews (e.g. asthma, COPD, heart failure, hypertension) and medication reviews of patients with polypharmacy and high-risk medications. The post holder will be also work towards meeting aspects of the Quality and Outcomes Framework and the local incentive schemes, maintaining CQC standards and improving medicines safety for our patients. There are opportunities to provide leadership on quality improvement and clinical audit. You will provide face to face and telephone consultations to patients and be on hand to answer medication related queries from colleagues and patients.

MAIN DUTIES AND RESPONSIBILITIES OF THE POST



OVERVIEW and SPECIFIC RESPONSIBILITIES



Clinical Pharmacy Service (CPS) within WISE 2 Primary Care Network. The post holder will be predominantly based as Sunny Meed Surgery; this may also include branch sites Greenfield and Goldsworth Park.

The clinical pharmacy service will be primarily patient facing and clinical in nature focused on improving medicines optimisation and safety working as part of a multidisciplinary team. Main elements include:

Management of chronic diseases such as asthma, COPD, diabetes, heart failure, hypertension. Working towards Quality Outcome Framework. Contribute to the Greener NHS programme, promoting greener use of medications. Working along side the duty doctor to manage on the day queries about medications, ensuring safe and effective prescribing, following local and national guidelines. Undertaking structured medication review and optimisation to tackle over-medication of patients, in particular elements will include: Inappropriate use of antibiotics Withdrawing medicines no longer needed Structured medication reviews to support patients to take their medicines to get the best from them, reducing waste and promoting self-care. Dedicated focus on priority groups: High risk and addictive medications Polypharmacy Care Home residents Frail elderly Patients with complex needs taking large numbers of different medicines. Management of medicines after discharge from hospital, reconcile medicines following discharge from hospitals and intermediate care including identifying and rectifying unexplained changes and manages these changes without referral to a GP. Promoting safe and evidence-based prescribing within the PCN Leading on the implementation new guidelines and recommendations on medicines use. Manage the clinical aspects of shared care protocols, clinical research with medicines and, liaison with specialist pharmacists Supporting the practice to meet CQC standards around prescribing and medicines. Acting on medicines safety alerts and managing medication shortages. Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individualised patient reviews. Put in place changes to reduce the prescribing of these medicines to high risk patient groups. Ensure that patients are referred to the appropriate healthcare professional for the appropriate period of time e.g. acute conditions, minor ailments, long term condition reviews etc. Supporting the delivery of the pharmacy and medicines optimisation elements of the network service specifications within the Network Contract DES with a particular focus on the structured medications review and optimisation specification and the enhanced health in care homes specification. Undertake and complete the Primary care Pharmacy Education Programme and independent prescribing if required Support the delivery of the Medicines Safety agenda and medicines related QoF quality improvements Collaborate with pharmacy colleagues across the different sectors to ensure medicines optimisation across the interface. Supporting further integration of general practice with the wide healthcare teams (including community and hospital pharmacy) to help improve patient outcomes. To assist in the training of pharmacy staff and other healthcare workers Be involved in clinical pharmacy training/teaching to pre-registration pharmacists, technicians and junior pharmacists. Provide pharmaceutical education and training to other healthcare staff in all areas to which you have a responsibility. Take part in the system wide education and training programmes for all healthcare staff. Provide education and training to primary care healthcare team on therapeutics and medicines optimisation. To follow all Health and Safety standards in compliance with the practice and PCN policies and procedures. This includes: Report all accidents, incidents and near misses Dealing with hazardous chemicals (including cytotoxic drugs) used in the department, following COSHH regulations.
Carry out the following in all areas of work to:

Develop, review and follow all standard operating procedures (SOPs) accurately. Maintain accurate manual and computer records. Housekeeping duties Ensure professional and legal standards are adhered to To participate in specified interface project/audits, according to need that supports medicines optimisation activities. The definition of these projects will link to issues arising at PCN level and actions arising out of the medicines safety QoF QI pertaining to interface matters.
All post holders:

Will be a member of the North West Surrey Clinical Pharmacy Network which meets bimonthly Will be supported by a senior clinical pharmacist and by GP Clinical supervisor To maintain registration, as a pharmacist, with the General Pharmaceutical Council, and to act in accordance with the code of ethics. The post holder agrees to participate in Continuing Professional Development (CPD) including further training and learning in order to develop the knowledge, understanding, skills, attitudes and values to become competent in the duties described. To undertake peer review and competency based assessment to ensure required standards of service are maintained.

M.Pharm degree in Pharmacy (4 years vocational degree).

1 year post graduate training followed by professional entrance exam.

Professional registration with the General Pharmaceutical Council.

Specialist knowledge acquired through post-graduate diploma level or equivalent training / experience (7)

Independent prescriber qualification or willing to work towards

Desirable:

Independent Prescriber (7)

Membership of the Royal Pharmaceutical Society (RPS)

A member working towards Faculty membership of the RPS
