An exciting opportunity has arisen within Cardiology for a Heart Failure and Cardiac Rehabilitation Trainee Advanced Clinical Practitioner (Band 7 leading to 8a on qualification).



The postholder will provide heart failure and cardiac rehabilitation expertise to support our innovative forward thinking acute and community teams within North Warwickshire. They will demonstrate strong leadership skills and will have the ability to drive services forward in its development including supporting our Acute Coronary Syndrome Cardiac Rehabilitation Teams.



This is a fantastic opportunity for the successful candidate to develop and enhance their clinical experience, skills and knowledge across both services whilst being supported by to complete your MSc is Advanced Clinical Practice.



The post holder will exercise advanced clinical skills, levels of judgment, discretion and decision making within both the Heart Failure and Cardiac Rehabilitation Teams and will act as a clinical expert in their field.



They will work collaboratively within both the Heart Failure and Cardiac Rehabilitation Teams, GE Home Virtual Wards, within Cardiology, with our Allied Health Professionals, Place Partners, Community Teams and Primary Care with a strong focus on continuing the services focus on supporting health inequalities within the local area.



Duties will include acting as a clinical expert and will work across acute and community services i.e. home visiting, supporting community/hospital clinics and the Cardiac Rehabilitation exercise classes. The post holder will have strong leadership and clinical skills to help support the Nursing Teams and Allied Health professionals to develop their skills within cardiology and will provide regular teaching sessions within the Trust to raise awareness of heart failure and cardiac rehabilitation.

Work autonomously an advanced practitioner within the speciality, managing a caseload of patient's delivering individualised direct patient care.



Direct responsibility for assessment, examination, investigation and diagnosis of patients within the area of work.



Provide visible and accessible professional leadership and demonstrate expert knowledge and standards of clinical practice.



Lead and develop a defined area of clinical practice within the designated area of practice, promoting interdisciplinary team and collaborative working practices.



Act as an expert educational resource for clinical staff, patients and carers by providing formal and informal education. Promote a positive learning environment throughout the Trust.



Act as a supervisor/assessor to staff members and students as required, providing educational advice and support. Ensure the wards/departments can access appropriate training support and guidance.



Participate and were required lead or assist in research projects as required, disseminating and ensuring the utilisation of research results to change practice.



Utilise research findings in the delivery of advanced clinical patient care, developing new ways of working.
