Job Summary
We are seeking a highly skilled and motivated Advanced Clinical Practitioner (ANP) to join our multidisciplinary team at Horley GP Surgery, delivering high-quality primary care services on behalf of a Primary Care Network (PCN). This role is pivotal in supporting the delivery of safe, effective, and person-centred care to a diverse patient population in Horley and surrounding areas. As an ANP, you will work autonomously to assess, diagnose, and manage patients with undifferentiated and complex presentations, contributing to the PCN's extended access and same-day care initiatives. Main Duties and Responsibilities
The ANP will provide comprehensive advanced nursing care within the scope of their professional registration, focusing on primary care delivery. Key duties include:
Patient Assessment and Management
: Independently assess, diagnose, and treat patients presenting with acute, chronic, and undifferentiated illnesses across all age groups, using advanced clinical assessment skills, history-taking, physical examination, and diagnostic investigations.
Prescribing and Treatment
: Prescribe medications safely and effectively as an independent prescriber, including managing minor ailments, long-term conditions, and health promotion interventions.
Triage and Same-Day Care
: Operate within a total triage system to prioritize and manage same-day appointments, urgent care requests, and home visits, ensuring timely access to services.
Multidisciplinary Collaboration
: Work collaboratively with GPs, pharmacists, practice nurses, and other PCN roles to develop care plans, contribute to MDT meetings, and support integrated care pathways.
Quality Improvement
: Participate in audits, service evaluations, and quality improvement initiatives to enhance patient safety and experience; mentor junior staff and contribute to training programs.
Administrative and Professional Duties
: Maintain accurate patient records in line with GDPR and professional standards; engage in continuous professional development (CPD) and adhere to NMC revalidation requirements.
PCN-Specific Contributions
: Support PCN-wide initiatives such as extended hours clinics, virtual wards, or population health management, as directed by the Clinical Director.
Qualifications and Training
Essential
:
Registered Nurse (Adult) with active NMC registration.
MSc in Advanced Clinical Practice (or equivalent Level 7 qualification), including modules in advanced health assessment, clinical decision-making, and non-medical prescribing.
Independent Prescriber qualification.
Desirable
:
Additional qualifications in minor illness, chronic disease management, or primary care-specific training.
Experience
Essential
:
Significant post-registration experience in primary or urgent care settings, including autonomous patient management.
Proven ability to work independently with clinical judgement and decision-making under pressure.

Experience in a PCN or multi-practice environment.
Prior involvement in triage systems or same-day access services.
