Bramley Village Health and Wellbeing Centre is currently looking to recruit a 30 hour per week Advanced Clinical / Nurse Practitioner to join our team at our Bramley site. We are flexible in regard to hours, however we do require one of the days worked to be a Friday.We are a multidisciplinary practice that includes 6 GPs, GP Registrar, 4 ANPs, Clinical Pharmacists, Practice Nurses and Health Care Assistants.The main duties of the role would include AM and PM clinics, seeing undifferentiated patients that are a mixture of pre-booked and on the day triaged appointments.Key responsibilities include the application of advanced clinical skills to assess, diagnose, treat, and advise patients within face to face and telephone consultations ensuring the use of evidence-based practice. The ANP/ACP will also lead in the development of new services and pathways to improve the quality of care and patient outcomes. This role requires dynamic clinical leadership and active participation in practice research activities to promote a culture of learning and continuous improvement within the practice.Main duties of the jobClinical Responsibilities:Patient Care: Deliver advanced and complex clinical care, providing assessment, diagnosis, treatment, and prescribing for patients with acute, chronic, and complex conditions.Autonomous Practice: Work independently within the scope of practice to manage complete clinical episodes, using advanced diagnostic skills and clinical judgment.Case Management: Manage patient cases from initial assessment to conclusion, including follow-ups and referrals, ensuring continuity of care.Prescribing: Utilise prescriptive authority responsibly following current guidelines and protocols to enhance therapeutic outcomes for patients.Diagnosis and Treatment Planning: Utilise clinical expertise to diagnose health conditions based on face to face and eConsultations. Prescribe medications, suggest lifestyle changes, or recommend further diagnostic tests as appropriate.Referral Coordination: Refer patients to other healthcare services when more detailed investigations or specialist care is needed, ensuring a seamless transition from virtual to in-person care.About usBramley Village Health and Wellbeing Centre is one of 3 sites across Leeds that are managed by the Fuller and Forbes Healthcare Group. Conduct comprehensive clinical assessments of patients using eConsultation tools. Evaluate patient symptom and medical history with any other relevant information provided through the digital platform. * Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based national and practice protocols.

• Undertake telephone consultations for patients presenting with an undifferentiated diagnosis to include consulting, assessing, diagnosing, implementing and evaluating treatment
• Provide interventions such as referrals and relevant prescribing
• Request and interpret tests appropriate to patients needs
• Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care
• Assess, identify and refer patients presenting with mental health needs in accordance with the national and local guidelines for Mental Health services
• Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment
• Communicate with and support patients who are receiving difficult or complex information or results in a professionally appropriate and supportive manner
• Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating
