The Advanced Clinical Practitioner (ACP) will work autonomously at an advanced level of clinical practice, providing assessment, diagnosis, treatment, and management of patients across the Primary Care Network.
The post holder will work across three GP surgeries, supporting delivery of high-quality, safe, and effective care, improving access for patients, and contributing to workforce sustainability. The ACP will demonstrate leadership across the four pillars of advanced practice: Clinical practice, leadership and management, education, and research/service development.
Key Responsibilities
1. Clinical Practice
Undertake comprehensive, autonomous assessment of patients, including history taking, physical examination, and interpretation of investigations
Diagnose, treat, and manage acute, chronic, and complex conditions within primary care
Independently prescribe medications (where qualified) and manage medicines safely and effectively
Manage same-day demand, urgent care presentations, and long-term conditions
Refer patients appropriately to secondary care, community services, or other professionals
Provide continuity of care and holistic, person-centred management
Manage risk, uncertainty, and complexity in clinical decision-making
Maintain accurate, clinical records using the practice clinical system EMIS
Contribute to safeguarding adults and children in line with local and national policies
2. Leadership and Management
Support multidisciplinary team (MDT) working across practices
Provide clinical supervision and support to junior clinicians and the wider team where appropriate
Contribute to quality improvement initiatives
Support workforce resilience by sharing expertise across all three surgeries
Participate in clinical governance, audit, and risk management activities
3. Education and Training
Support education and development of the primary care workforce, including trainees, nurses, paramedics, and physician associates
Act as a role model for advanced clinical practice
Participate in mentorship, preceptorship, and supervision where appropriate
Maintain own professional development in line with ACP frameworks and revalidation requirements
4. Research and Service Development
Contribute to audit, research, and evaluation of services within the PCN
Use evidence-based practice to inform clinical care and service improvement
Support implementation of national and local guidelines

Community services, secondary care clinicians, and allied health professionals

Qualifications and Training (Essential)
Registered healthcare professional (e.g. Nurse, Paramedic, Physiotherapist, Pharmacist) with current professional registration
Master's level qualification in Advanced Clinical Practice aligned with HEE / NHS England ACP framework
Independent Prescribing qualification
Evidence of advanced clinical assessment skills
Experience (Essential)
Significant post-registration clinical experience
Experience working in primary care, urgent care, or a relevant clinical setting
Experience of autonomous clinical decision-making
Experience working within a multidisciplinary team
