Assess, diagnose, and manage patients with acute and chronic conditions. Order, interpret, and act on investigations (blood tests, imaging, ECGs, spirometry). Prescribe and review medications independently. Perform minor procedures (wound care, sutures/staples, joint injections if trained).
Develop and implement individualised care and treatment plans. Manage long-term condition clinics (diabetes, hypertension, COPD). Provide health promotion, disease prevention, and lifestyle advice. Triage patients and decide on escalation or referral to GPs or specialists.
Provide patient education, self-management support, and follow-up care. Contribute to service planning and improvement initiatives. Support clinical audits and quality improvement projects. Support practice in implementing evidence-based guidelines and protocols (NICE, local policies).
Provide clinical leadership to junior staff, nurses, or other allied health professionals. Use data and insights to improve patient outcomes and service efficiency. Maintain accurate and detailed patient records. Ensure compliance with data protection, clinical governance, and safeguarding policies.
Participate in revalidation, CPD, and professional development activities. Contribute to multidisciplinary team meetings and case discussions.
