Job Description

The Advanced Clinical Practitioner will work at Eastham Group Practice to provide expert clinical care, supporting patients with a wide range of health needs. The role involves autonomous practice, advanced clinical decision-making, and collaborative working with the wider multidisciplinary team to improve access, continuity, and quality of care.Key Responsibilities Independently assess, diagnose, treat, and manage patients presenting with undifferentiated and undiagnosed conditions. Prescribe medications and request/interpret diagnostic investigations within scope of practice. Deliver care for patients with long-term conditions, frailty, and complex needs.



Support urgent care provision, including same-day access clinics and home visits where appropriate. Work closely with GPs, nurses, pharmacists, and allied health professionals within the practice. Contribute to service development, clinical governance, and quality improvement initiatives. Provide leadership and mentorship to junior staff and students.



Maintain accurate and timely clinical documentation in line with practice policies.Person Specification (Summary) Essential: MSc in Advanced Clinical Practice or equivalent qualification. Current registration with NMC/HCPC/GPhC. Independent prescribing qualification (V300). Proven experience in a primary care or urgent care setting.



Excellent clinical assessment and decision-making skills. Strong communication and team-working abilities. Desirable: Experience working across multiple practices or within a PCN. Knowledge of local health systems and community services.



Leadership or teaching experience.
