Job Description

Advanced Critical Care Outreach Practitioners are clinical professionals who have developed their skills and theoretical knowledge to a very high standard.



They are highly experienced and educated members of the critical care outreach team who can diagnose and treat health care needs or refer to an appropriate specialist if needed. They are empowered to make high-level clinical decisions and to work autonomously. The expertise of the Advanced Critical Care Outreach practitioner will be focused in supporting staff and caring for deteriorating patients across the whole hospital.



The post holder is expected be a clinical expert in assessment, diagnosis and treatment of patients who are unwell or stepped down from the Critical Care Units in LNWUH Trust.



The post holder is responsible and accountable for evidenced based practice for management of conditions for which the post holder will prescribe medicine as an Independent Non-Medical Prescriber



Clinical / professional : Contribution to a 24 hour per day, 7 day per week critical care outreach service. Support junior medical, nursing, and AHP staff to achieve training and development requirements. Undertake comprehensive clinical assessment of a patient's condition. Request, interpret and respond to laboratory and radiological diagnostic tests as required. Initiate and manage a clinical treatment plan. Support nurses in the management of mechanical ventilated patient at the CMH site, including monitoring blood gases and altering ventilation settings to ensure optimal ventilation on patients waiting to be transferred to a critical care unit. Provide accurate and effective clinical handovers. Assist with invasive interventions, such as arterial cannulation and central venous access within the scope of practice. Provide professional leadership and support within a multi-professional team Work autonomously in recognised situations. Demonstrate comprehensive knowledge across a range of subject areas relevant to the field of critical care. Attend cardiac arrests/emergencies in UCC. Critically analyse, evaluate, and synthesise different sources of information for the purpose of assessing and managing the care of a critically ill patient.
