Advanced Nurse Practitioner (ANP) Role Overview: The Advanced Nurse Practitioner (ANP) will provide high-quality, patient-centred care within the practice. The ANP will utilise advanced clinical skills to assess, diagnose, and manage a wide range of health conditions, working autonomously and collaboratively within a multidisciplinary team. Key Responsibilities: Clinical Assessment and Diagnosis: Conduct comprehensive patient assessments, including history taking and physical examinations. Diagnose and manage acute and chronic health conditions.
Treatment and Management:
Develop and implement treatment plans, including prescribing medications and therapies. Provide expert advice on disease prevention and health promotion. Patient Care and Support: Offer patient education and counselling to support self-management and informed decision-making. Coordinate care with other healthcare professionals to ensure continuity and quality of care.
Referral and Collaboration:
Make referrals to specialists and other healthcare services as needed. Work collaboratively within a multidisciplinary team to deliver integrated care. Professional Development: Engage in continuing professional development to maintain and enhance clinical skills and knowledge. Participate in clinical audits, research, and quality improvement initiatives.
Leadership and Mentorship:
Provide leadership and mentorship to junior staff and nursing colleagues. Contribute to the development and implementation of practice protocols and policies. Qualifications and Experience: Registered Nurse with a valid NMC PIN. Advanced clinical skills and experience in a primary care setting.
Independent prescribing qualification is essential.
