Job Summary



We are seeking a highly motivated and experienced

Advanced Nurse Practitioner

to work within our well-established and supportive general practice team. You'll play a vital role in offering expert clinical care, managing both acute and chronic conditions, and driving preventative health strategies. This is an ideal opportunity for someone passionate about leadership in primary care and keen to make a real difference in community health.

We are looking for an ANP to work

Monday to Friday

, with

one on-call day per week

. On-call duties include

seeing patients

reviewing and actioning laboratory results

signing prescriptions

supporting urgent clinical queries

You'll have a mix of face-to-face and telephone consultations, and autonomy to manage minor illnesses, acute presentations, and long-term conditions within your scope of practice. The role will be supported by our clinical team, with regular team meetings, CPD opportunities and access to further training to help you develop your career and skills.

What We're Looking For



We're seeking a qualified ANP with experience in general practice or urgent care settings, confident in managing a broad range of clinical presentations independently. Someone who works well as part of a team, contributes to shared goals, and is eager to grow professionally within a progressive, patient-focused setting.

Primary Key Responsibilities



The following are the core responsibilities of the Advanced Nurse Practitioner. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

1. To manage patients presenting with a range of acute and chronic medical conditions, including undifferentiated undiagnosed conditions, providing subject matter expert advice

2. To identify red flags and underlying serious pathology and take appropriate action

3. To use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice

4. To implement and evaluate individual specialised treatment plans for chronic disease patients

5. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patient's health

6. To provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

7. To assess and triage patients, including same day triage, and as appropriate provided definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team

8. During

1 day/week on call duties

, to provide clinical decision-making and care for

on-the-day urgent cases

, including telephone triage, face-to-face consultations, or home visits as necessary

9. To actively take a personalised care approach and population centred care approach to enable shared decision making with presenting patients

10. During

1 day/week on call duties

, to respond to and manage urgent medical queries from patients or clinical staff

11. To provide wound care (ulcer/Doppler, etc.)

12. To provide specialist clinics such as ear syringing, contraception, etc.

13. To undertake the collection of pathological specimens

14. To provide travel medicine services

15. To request pathology services as necessary

16. To process and interpret pathology and other test results as required in a timely and clinically appropriate manner (especially

during 1 day/week on call duties

)

17. To provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

18. To develop, implement and embed health promotion and wellbeing programmes

19. To maintain accurate and up-to-date clinical records in line with practice and regulatory standards as well as in conjunction with extant legislation

20. To ensure SNOMED CT codes are used effectively

21. To maintain chronic disease registers

22. To develop, implement and embed well woman clinics

23. To develop, implement and embed well man clinics

24. To develop, implement and embed travel clinics

25. To develop, implement and embed an effective call/recall system

26. To be responsible for the implementation of an effective immunisation programme

27. To chaperone patients where necessary

28. To prioritise health issues and intervene appropriately

29. To support the team in dealing with clinical emergencies

30. To recognise, assess and refer patients presenting with mental health needs

31. To implement vaccination programmes for adults and children

32. To review and process clinical documents

during 1 day/week on call duties

, ensuring accurate workflow and appropriate coding or follow-up actions

33. To be an extended and supplementary prescriber, adhering to extant guidance

34. To support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required

35. To contribute to practice targets (QOF, etc.), PCN targets and service requirements (IIF, etc.) complying with local and regional guidance

36. To complete all relevant training in order to provide multi-professional clinical practices and CPD supervision to other roles within primary care, for example first contact practitioners and the personalised care roles.

37. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)

38. To delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)

39. During

1 day/week on call duties

, to collaborate with the wider clinical team to ensure, safe, effective and efficient care delivery.

40. To support the clinical team with all safeguarding matters in accordance with local and national policies

41. To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

42. To deliver opportunistic health promotion where appropriate

Secondary responsibilities:



In addition to the primary responsibilities, the Advanced Nurse Practitioner may be requested to:

1. Act as the audit lead, effectively utilising the audit cycle

2. Act as the IPC lead for the practice, ensuring compliance, undertaking audit as necessary

3. Monitor and ensure the safe storage, rotation and disposal of medicaments

4. Undertake overall management of the nursing team, providing guidance when necessary, acting as a mentor to students and newly qualified staff

5. Maintain an effective nursing staff rota ensuring all clinics are staffed appropriately

6. Participate in local initiatives to enhance service delivery and patient care

7. Support and participate in shared learning within the organisation

8. Develop an area of specialist interest, taking the lead within the organisation

9. Continually review clinical practices, responding to national policies and initiatives where appropriate

10. Participate in the review of significant and near-miss events applying a structured approach i.e., root cause analysis (RCA)

11. Drive the development of nursing services within the organisation, liaising with external agencies and professional organisations as required

12. Develop practice administrative and clinical protocols in line with the needs of the patient and current legislation
