Chawton Park Surgery is recruiting for a 30-hour Advanced Nurse Practitioner to join our Triage and Research Team.
Our new team member will work both independently and within a multi-disciplinary team environment, to provide safe, holistic care to our patients. This will include remote and face to face appointments, reactive and proactive home care visits. You will be required to work within your professional competence and boundaries to provide both urgent and planned care to patients. You will need to prioritise and triage the needs of patients, making any necessary referrals for investigations or to secondary care in the appropriate manner.
The Practice is a large Commercial Research Practice, where we have a growing research team that provide research across our PCN, and we are looking to expand this further. This role will also be part of this growing research team.
ANPs in primary care can develop close, long-term relationships with their patients and work in partnership with them to achieve optimum health. They are autonomous in making decisions based on assessment, diagnosis and the interpretation of test results. They can independently prescribe appropriate medication, evaluate or refer to other specialists if necessary.
