Job Title Advanced Nurse Practitioner Responsible To Dr Rushworth GP Partner and Clinical Practitioner Lead Clinically Practice Manager Administratively Job Summary The Advanced Nurse Practitioner ANP provides expert care across the patient journey assessment diagnosis treatment evaluation within professional boundaries. Working autonomously and collaboratively the ANP uses clinical decision-making and critical thinking to support practice policies and patient care delivery Key Responsibilities Clinical Practice Assess diagnose plan and evaluate care for patients with undifferentiated or complex needs Provide holistic clinical assessments physiological and psychological Manage acute chronic conditions integrate drug and non-drug treatments Prescribe and review medications per evidence-based protocols Support patient adherence to treatments and promote self-care Prioritise urgent care and initiate emergency treatment as needed Undertake home visits when required Support health promotion and immunisation such as Flu or COVID Deliver evidence-based care in line with NICE and national guidelines Participate in on call rota Support with total triage system Communication Use clear sensitive communication and ensure informed consent Adapt communication methods to suit understanding and culture Act as advocate and signpost support services Maintain accurate legal clinical documentation Quality and Governance Practice within NMC code and legal frameworks Maintain accurate records and organise own workload Deliver care aligned with Public Health England and NICE Evaluate care effectiveness via audit peer review and feedback Support QOF IIF DES CQC monitoring and compliance Manage clinical incidents using structured frameworks Apply safeguarding principles and raise concerns appropriately Leadership and Development Take responsibility for personal CPD and supervision Mentor and support development of others Foster a learning culture and share innovations and updates Lead or support changes in service delivery or policy Help develop and implement practice guidelines and protocols Team Working Work effectively within and across teams Delegate safely and support colleagues development Manage defined services and projects and set measurable outcomes Promote shared decision-making to improve care Risk Management Identify and mitigate clinical and environmental risks Supervise safe medication storage disposal and infection control Ensure a safe compliant working environment Translate national policies into effective local strategies Information Management Use technology and software for planning monitoring and reporting Apply accurate clinical coding for audits and data retrieval Handle FOI requests appropriately Communicate data insights such as prescribing referrals finance to team Learning and Education Mentor junior staff and assess clinical competence Share knowledge gained from training conferences and updates Identify own learning needs and undertake appropriate training Educate patients carers and staff in a supportive environment Equality and Diversity Promote non-discriminatory inclusive care Challenge discrimination and support diverse patient needs Ensure adherence to chaperone policies and support marginalised patients Note This job description may be amended following review and consultation.
