Make professionally autonomous decisions within scope of role and professional boundaries. Recognise and work within own competence and professional code of conduct as regulated by the NMC. Produce accurate, contemporaneous and complete records of patient consultations in line with legislation, policies and procedures. Work as part of a multidisciplinary team delivering urgent/same day care to patients.
Provide clinical triage services with respect of online requests and telephone contacts. Supporting care navigators to gather information necessary for clinical prioritisation. Work collaboratively with Duty Team to assess, diagnose, plan and evaluate treatment/intervention and care for patients presenting with undifferentiated diagnosis. Assess, diagnose and treat patients in practice, remotely or in their own home.
Conduct comprehensive assessments of patients presenting with acute, chronic and complex conditions. Requesting of diagnostic investigations and interpret within agreed protocols. Recognise, assess and refer patients as appropriate using practice approved pathways, portals, protocols, ensuring referral meets the referral criteria set. Provide advanced, specialist care to patients as required in accordance with clinical based evidence, NICE and NSF.
Prescribe and review medication appropriate to patient needs and in accordance with evidence-based practice and national protocols. Contribute to local and national practice targets clinical remit (ie QoF, CEIF, prescribing incentives and national benchmarking). Participate in clinical audit, quality improvement and research initiatives. Engage in continuous professional development and maintain professional registration requirments.
