Advanced Pharmacist Transfer To Assess/complex Discharge Services

Job Description

To provide a highly specialised clinical pharmacy service. Co-ordinate with the multidisciplinary teams to identify patients requiring pharmacy input To ensure the pharmaceutical service facilitates early discharge where appropriate To plan the day-to-day delivery of the pharmacy service to the Bluebell unit. To work across traditional boundaries e.g. primary / secondary care to provide seamless care.



To use the skills as a pharmacist prescriber to write discharge prescriptions to meet service needs To participate in the pharmacy departments on call and out of hours pharmacy services rota as and when required. The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trusts policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately. If successful this vacancy will require an enhanced DBS check at a cost of 54.90. This cost will be deducted from your first Trust salary In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of 16.
