We are looking for motivated and ambitious Advanced Practioner with a passion for enabling people in Doncaster to live in the place they call home with the people and things that they love, in communities where they look out for one another, doing things that matter to them.
You will play a key role in the development and implementation of the locality-based approach, using creative ways to support people to live the lives they choose, helping them to stay in control whilst improving their confidence, connecting them with their local communities and supporting them to flourish.
As an Advanced Practitioner within our team, you will promote and embed our practice framework which is based on strong, legally literate, and rights-based practice. The role will involve supporting social care practitioners with undertaking holistic, good conversations with adults, carers, and families to help them make informed choices and decisions based on what matters to them. Undertaking formal supervisions and providing daily practice support and guidance as part of a duty function within the team.
