What does a Social Worker do at Hillingdon?

Permanent recruitment of an Advanced Social Work Practitioner is required to work within ASC Hospital team to support and facilitate hospital discharges.

The role is to support hospital discharges for our residents from Hillingdon Hospital as well as out of borough hospitals, rotating on a duty rota and to carry a small caseload of complex cases.

Key Responsibilities:

  • To act as Duty manager
  • To manage daily discharges via Bridging pathway and Pathway 3 placements
  • To triage cases alongside the Hillingdon HUB and out of borough hospital referrals
  • Attending conference calls with health professionals and being able to deliver results
  • Supporting team members, allocating cases, management of stepdown beds
  • Safeguarding Manager role, attending regular meetings with safeguarding lead at THH on progression of investigations
  • Carry a small caseload of complex cases

The successful candidate will be committed to providing quality services that promote independence/wellbeing, control, choice and social inclusion. You will demonstrate a person centred approach by keeping the service user at the centre of the assessment and discharge process.

We need an enthusiastic person to join the hospital team, to take part in managing safe hospital discharges in a busy environment.
