This is a brand new post within the Health Community Support Team, for the Health Board.
This service will provide intermediate care, support and rapid access to mutli professional intervention in order to support patients from discharge from hospital and avoid unneccessary admissions into hospital.
You will be operationally managing the AHP resource within this service, which will work across the Health Board.
The post holder is identified as a highly skilled, senior practitioner with expert knowledge in Community and Intermediate Care.
The post holder will provide strategic and operational leadership for AHP's working within the service.
The post holder will work with the community, primary care and admission avoidance teams across the health board and local authorities to respond in a timely way to urgent patients.
The post holder will take a lead on the continuing development of the Intermediate Care Service and its strategic and operational planning.
The post holder will undertake research and development on an ongoing basis, leading clinical audit and research associated with the Intermediate Care Service.
