Job Description

The main duties of the role are:

Conducting home visits for patients who are unable to attend the practice (e.g. frailty, palliative care, acute illness, complex, chronic long-term conditions) Assess undifferentiated patients with a range of acute, non-acute and chronic medical conditions. Take history, make appropriate physical examinations, formulate differential diagnoses and carry through management treatment plans when working in a home visit environment. Through telephone triage and clinical assessment, obtain a relevant and sequential history of the presenting complaint to determine a working diagnosis.



Ordering and interpreting blood tests, scans, x-rays, including other relevant investigations and referrals Recognise the need and initiate emergency treatment to save a life or safeguard the patient's interests, and refer to emergency services when appropriate. This includes escalation/referral to hospital or other community services. To be professionally and legally responsible and accountable for all aspects of clinical decision-making within your clinical competence. Working alongside other ANP/ACP/GP's, community teams to provide holistic patient care.



Escalating complex cases to senior clinicians Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual).
