Provide support to general practice staff by managing prescription and medication-related queries Repeat prescription process Acute prescription requests Medicines reconciliation during transfers of care Maintain safe prescribing systems and provide expert clinical medicines advice, ensuring the public and social care needs of patients within the GP practices are met. Patient facing clinical medication reviews, including structured medications reviews Providing answers to medicine-related queries from GPs and other healthcare teams Management of common/self-limiting ailments this includes signposting to community pharmacy or referring to GPs and other healthcare teams where appropriate. Risk stratification using clinical reports, identify cohorts of patients at risk of harm from medicines.
