We are currently looking for a Social Worker for Monmouthshire Council\'s new project.

12 month post with possibility of extension subject to funding. Full time post.

\xe2\x80\x9cGoing Home, Staying Home\xe2\x80\x9d - Our Purpose:-

It has long been recognised that children who are raised in their own families have better outcomes in life. However even when children can not stay in their birth families this does not mean that there will not come a time when risks, and needs change and can be managed more sustainably. Going Home and Staying Home are a project team who will be supporting the case holding social workers to assess families and children currently in the Looked After system, to identify needs and risks, and offer families support to meet the children\'s need and rehabilitate them home to their communities

The Purpose of this Role:-

To work alongside the Senior Practitioner to undertake assessments of children who have been in the Looked After system, and their families to understand the changes within family structure and dynamics, to consider the current and potential future needs of the children and young people, and to consider their longer term options for care and support. Where the plan is rehabilitation to develop a child and family focused, strengths based care plan with support from other agencies to manage the process of not just the journey home, but to support the ups and downs of a child\'s journey

Expectation and Outcomes of this Role:-

  • To develop care plans with young people, children and families which will build on strengths, mitigate risks and continue to have a longer term vision of life post care at what ever age this occurs;
  • To direct and work along side dedicated Going Home, Staying Home support workers to find creative and individual ways to increase communication and trust with children, young people and their families in order to find a way to really hear what young people want and how you can work together to move toward these goals;
  • wherever safe and possible establish a route home for children so that they can live within their family and community, and out of the care system
  • Build relationships with children, young people, their family and carers so that you can understand their needs and complexities in meeting those needs long term
  • Assess and work with multi-agency partners and project support workers to reduce the level of risk and vulnerability of children and young people.
The outcomes achieved by this role are for children and young people:

  • To achieve happiness
  • To return home to their families and communities where possible and to understand why, if this is not possible, and what would need to change to enable their return;
  • Where not possible to return home to be supported to achieve their potential and to live independently as an adult.
Your responsibilities are to:-
  • To work with the case holding social workers, support workers and the Placement Team to fully understand the child or young persons
  • Be confident in presenting information in formal meetings and at court.
  • Work in a flexible and imaginative way to provide support and advice for children, young people, their families and carers.
  • Complete proportionate and quality assessments (i.e parenting assessments, placement with parents reports etc) of individuals and families; analysing strengths and risks to inform short to long term care planning for children across the service.
  • Formulate plans to meet assessed needs and risks building on strengths of children, families and carers.
  • Implement agreed plans to safeguard children and to increase the capacity and resilience of their parents and/or carers
  • Chair reviews/meetings
  • Work as part of a team and contribute to the development of the service
  • Form part of an out of hours oversight rota alongside the management team, to support GHSH staff who are working out of hours
  • Initiate, develop and sustain effective working relationships with partner agencies.
  • Support with the monitoring of safe working arrangements for the Family Support Workers by being part of a service rota for out of hours working in line with the FSW\'s working between 9am -10pm 7 days a week.

Required -

Social Work Degree (or equivalent)

Social Care Wales Registration

Enhanced DBS on Update Service (can help with this if needed)
