Job Overview



The Community Support Assessor provides floating support and enablement services to Croydon residents aged 18+ with disabilities, learning disabilities, and/or autism who have lower-level needs. This role focuses on improving outcomes for individuals through a person-centred approach, enhancing independence, and promoting community integration.

By working closely with a great local team to develop skills and knowledge and share good practice as well as coaching and mentoring for new or inexperienced CSW's and support to volunteers.

By providing support to the Community Support Coordinator or Area Manager as requested.

Key Responsibilities



Assessment and Support Planning

Conduct comprehensive assessments of individuals' needs, strengths, and goals.

Develop personalized support plans to address identified needs and promote independence.

Regularly review and update support plans to ensure they remain relevant and effective.

Housing and Tenancy Support



Assess and mitigate risks of homelessness and tenancy breakdown.

Assist individuals in transitioning to more independent living arrangements.

Provide guidance on tenancy issues and support with housing-related matters.

Educate clients on home maintenance and independent living skills.

Assist in reporting repairs, antisocial behaviour, and other issues to landlords.

Community Integration and Safety



Facilitate connections with community groups to enhance independence and social networks.

Conduct travel training to improve clients' mobility and independence.

Establish links with local crime prevention groups.

Educate clients on personal safety within the home and community

Financial Management and Support



Assess clients' financial situations and develop budgeting plans.

Assist in setting up direct debits and accessing relevant benefits.

Educate clients on household bill responsibilities and financial management

Provide training on preventing financial abuse in the community and online

Employment and Education Support



Assess clients' educational and vocational needs and aspirations

Facilitate access to volunteering, training, and employment opportunities

Signpost individuals to appropriate educational resources, including IT and basic skills training

Support clients with low literacy levels in understanding correspondence

Health and Wellbeing



Assess clients' health and wellbeing needs

Support the development of life skills to maintain independence and wellbeing

Assist in accessing healthcare services, including GP appointments and medication management

Conduct regular welfare checks

Implement trauma-informed practices in service delivery

Safeguarding and Risk Management



Identify and report any safeguarding concerns following organizational procedures

Conduct risk assessments and implement appropriate risk management strategies

Service Delivery and Transition

Provide person-centred support tailored to individual needs and goals

Gradually reduce support intensity as clients develop greater independence

Plan and facilitate smooth transitions for clients moving to more independent living situations.

Supervisory Responsibilities



Supervise a team of Community Support Workers, providing guidance, support, and mentorship

Conduct regular one-on-one meetings and performance evaluations with team members

Identify training needs and facilitate professional development opportunities for staff

Allocate and manage caseloads for Community Support Workers

Ensure equitable distribution of work and monitor individual workloads

Provide support and guidance on complex cases

Identify and nurture talent within the team for career progression

Support the induction and training of new team members

In addition to the responsibilities outlined above, you may be required to perform other duties as assigned by management from time to time.

Role modelling to CSWs and volunteers how we work with Members, how we complete records and how we use the systems provided.

Job Types: Full-time, Permanent

Pay: 24,420.00-25,912.59 per year
