An exciting opportunity has arisen for a Band 4 Assistant Practitioner (Nursing) based in the Pembroke District Nursing Team.
The role will allow you to support the management of a delegated caseload within the Community Nursing Team providing high quality and complex care management to patients in their own homes, with the guidance and support of an experienced District Nurse Team Leader.

This is a permanent full time position worked 37.5 hours per week over a 7-day period. The post holder will need the ability to travel between sites in a timely manner.

The post holder will ensure high standards of care are delivered to patients and their families/carers within the home or clinic environment.
Duties will include wound care/management, administration of medication, venepuncture, catheterisation and continence promotion, holistic re-assessment, recording of observations and health promotion. The ability to recognise an unwell patient and how to escalate care accordingly is essential, as is the ability to effectively manage your daily caseload.
The role includes liaising with staff, patient/carers and other agencies/organisations as required to ensure appropriate delegation of duties and risk management, whilst ensuring effective communication between those involved in the care package.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
