This service will provide falls advice, assessment, and intervention when needed at local venues. The service that looks at both prevention and intervention - expanding on some exciting work already going on in CTM and making this available to our whole community. The team will work with an individual and their carer to make decisions about the best way to reduce the risk of the person falling, raise awareness of risk factors for falling and make any necessary changes to prevent falls. We are creating innovative offers of assessment, advice and intervention, building strong integrated teams and designing a seamless pathway for our patients with our patients. Support Allied Health Professions (AHP) namely Physiotherapists (PT) and Occupational Therapists (OT) in the delivery of AHP intervention for patients in the community, to prevent and manage falls. To work as part of the CTM Falls Prevention and Management Team to undertake specific clinical and administrative tasks, liaise with health colleagues, Local Authorities and third sector organisations to ensure care for patients within the CTMUHB/ Bridgend/RCT/Merthyr County Boroughs as instructed and delegated by the AHP, responding flexibly to the needs of the service. Support the AHP staff by assisting with all modalities of therapeutic care focused toward maintaining the patient within the community and the prevention of avoidable falls and subsequent effects.
