Key Responsibilities:
Support the assessment, planning, and delivery of individualised care for assigned service users within your competency scope.
Work collaboratively with the multidisciplinary team to ensure continuity and high standards of care.
Communicate and report accurately on changes in service users' conditions.
Promote service user choice, participation, and empowerment, ensuring their views are considered in care delivery.
Maintain safe working practices and confidentiality at all times.
Support and mentor junior or new staff as required.
Uphold organisational standards and challenge unsafe or inappropriate practices using proper channels.
Requirements:
Level 5 Foundation Degree in Assistant Practitioner (Healthcare) or equivalent.
Experience in healthcare; experience in specialist services such as neurology or palliative care is desirable.
Knowledge of relevant legislation affecting clinical practice.
Experience working in a clinical setting within a multidisciplinary team.
Strong verbal and written communication skills.
Excellent interpersonal skills with the ability to build effective working relationships.
Ability to respond appropriately to service users in distress.
