Key Responsibilities
Support the assessment, planning, and delivery of individualised care under the direction of a registered professional.
Work collaboratively with nurses, therapists, and other members of the multidisciplinary team to provide consistent, high-quality care.
Maintain high clinical standards by working within your scope of practice and following all national guidelines and policies.
Communicate effectively with team members, providing timely updates on changes in service users' conditions.
Promote choice, dignity, and person-centred care, ensuring that service users and their families are actively involved in decisions.
Support a culture of openness and safety by challenging inappropriate practices and using internal reporting procedures when necessary.
Contribute to the safe and efficient running of the service, maintaining confidentiality and professionalism at all times.
Participate in audits, training, and quality improvement initiatives to support ongoing development and service excellence.
About You
Level 5 Foundation Degree in Assistant Practitioner (Healthcare).
Experience working in healthcare, ideally within a specialist setting such as neurological or palliative care.
Strong knowledge of healthcare legislation and its application in clinical settings.
Experience working as part of a multidisciplinary team.
Excellent communication and interpersonal skills with the ability to build positive relationships with service users, families, and colleagues.
Compassionate, adaptable, and able to respond calmly to people in distress.
