Are you a highly skilled Assistant Practitioner looking for a new opportunity to work in an organisation that will help you develop and achieve your goals? We are offering an exciting opportunity to join our Integrated Mental Health Team (IMHT) based across the Tamworth area.



The Integrated Mental Health teams work as part of a 'neighbourhood' offering place-based provision of mental health services in partnership with other local organisations. Clinicians within the IMHT will offer interventions and treatments which are in line with NICE guidelines.



As Assistant Practitioner you will work as part of a multidisciplinary team to support the delivery of high quality patient/client care through completing assessments/reviews and feeding back to clinical leads and staff within the integrated mental health team.



You will be supported by a friendly team and supported and encouraged to undertake Continuing professional development opportunities.



You will be required to travel over the Tamworth area.

This role is not eligible for visa sponsorship.



As Assistant Practitioner you will work under the supervision from a registered professional to provide high quality care and treatment as a member of the multi-disciplinary team, informed by excellent communication, analysis of information, planning, and organisational skills.



As part of this role, you will:



Assist with managing a caseload of service users with secondary mental health needs.

Develop treatment plans and using assessment tools to inform formulation and the decision-making process.

Work at times autonomously and making decisions with recovery of the individual at the centre of that process.

Liaise with family members to inform the assessment and formulation of care plan and support provided.

Provide support and spending therapeutic time with an allocated group of service users referred to the IMHT.

Assist the named worker to assess, plan, implement and evaluate individualised care plans and care pathways.

Promote service-users' wellbeing and to assist them to maintain their place in the community or manage transitions between services.
